Understanding Levels of Prevention: Primary, Secondary, and Tertiary

Substance use prevention is not a single action. It is a continuum. People enter that continuum at different points in their lives, and prevention strategies must meet them where they are. In the field of public health and behavioral health, prevention is commonly divided into three levels: primary, secondary, and tertiary. Each level reduces risk differently.

Primary Prevention (Before Use Begins)

Primary prevention focuses on reducing the likelihood that substance use will start in the first place. This work often targets youth, families, and communities before experimentation or early use begins. Examples include education, skill-building, positive youth engagement, clear parental boundaries, and community environments that reduce risk.

The goal of primary prevention is to lower the chance of first use by strengthening protective factors and reducing exposure to risk factors.

Secondary Prevention (After Early Use Starts)

Secondary prevention aims to intervene early once substance use or experimentation has already begun, before it progresses into a harmful pattern. This includes brief interventions, screening, early referrals, and supports that help a person change course before addiction develops.

The goal at this level of prevention is to prevent escalation and reduce harm after early or occasional use begins.

Tertiary Prevention (When Use Is Already Causing Harm)

Tertiary prevention supports people who are already experiencing substance use disorder or related harms, helping them stabilize, reduce risk, and make movement toward recovery. This includes overdose prevention, harm-reduction tools like Narcan, access to treatment, recovery coaching, and long-term supports.

The goal at the tertiary prevention level is to reduce harm, increase safety, and support recovery for those already affected.

Primary Prevention

As an example of our primary prevention efforts, ECO provides support and outreach to youth programs, such as PASS and the Youth Leadership Academy. Our work involves awareness, education on protective factors, and stigma reduction.

Secondary Prevention

This stage involves early intervention when police, schools, or other community partners observe emerging substance use concerns. ECO accepts referrals for individuals of all ages, with a focus on helping communities identify and support anyone who may be at risk of substance use or the development of a substance use disorder, including adolescents.

Tertiary Prevention

Tertiary prevention includes recovery coaching, post-overdose outreach, harm reduction, and connection to treatment and supports. This is where ECO conducts a large portion of our efforts.

By responding across all stages, ECO works with our fellow community programs and partners of the county to reduce the overall impact of substance use. We’ve been saying it all October for #PreventionMonth, and we’ll say it again: prevention is not a single moment. It is a system. Reducing risk at any stage can make a real difference.
